    If you are using a gen1 (square) console with v1 controllers along with an external USB3 drive for games, there is a hardware fault on the first gen systems where USB3 emits an RF frequency thats super close to the Bluetooth frequency of the V1 Console/Controller. Try unplug the Hard Drive and test. If that solves the issue, find a triple shielded USB3 cable @ 1.5 to 2m long and move the drive as far away from the console as possible.

    That was my issue at least, so I pass this info on where I can. Hope it helps
